Our Firm, MNT Tan\u0131 ve Tedavi Merkezleri Ticaret A..\u015e. (Neolife), takes necessary measures to protect and ensure security of your data. Therefore, we have issued this Information Text pursuant to Procedures and Principles to Follow for Meeting Obligation of Informing and to meet requirements of the Personal Data Protection Law No. 6698 and to inform you about how we protect your personal data (hereinafter referred to as \u201cInformation\u201d) and how we use your information.<\/p>\n
Personal information implies the information that determines your identity details and\/or make such details determinable. Processing the personal information implies \u201cany and all processes performed on personal information, including obtaining, recording, storing, and changing personal information as well as sharing thereof with third parties and transferring the information to other countries with automated processes or non-automated processes that are already a part of any data recording system\u201d.<\/p>\n
We obtain your personal data by verbal, written, visual or electronic means via Call Center, internet, mobile applications, physical spaces and over the phone depending on the essence of service rendered to provide high standard services for you.<\/p>\n
Neolife (the \u201cData Controller\u201d) processes your information in line with relevant legislation, including the Personal Data Protection Law. Neolife also takes appropriate technical and organization security measures to protect your information against accidental loss, misuse, unauthorized access, changes and damages.<\/p>\n
Purpose and Legal Basis of Data Processing<\/b><\/p>\n
Your personal data and your sensitive personal data are processes and stored pursuant to your explicit consent and to the extent it is required to deliver all medical diagnosis, examination, treatment, consultation and care services, it is clearly obligated by laws, it is directly related with entering into any agreement or fulfilling contractual obligations, it is mandatory for the data controller to fulfill a legal obligation, it is required to set, use ad protect a right and it is necessary to perform medical diagnosis, treatment and care services.<\/p>\n
Your above mentioned personal data and your sensitive personal data can be processed in line with the Law for below stated purposes;<\/p>\n
\u2022 Protection of public health, and carrying out protective medicine, medical diagnosis, treatment and care services,<\/p>\n
\u2022 Sharing the information that is requested by Ministry of Health and other state institutions and organizations pursuant to relevant legislation,<\/p>\n
Verifying your identity details by relevant units of Neolife; financing your healthcare services and paying costs of examination, diagnosis, consultation and treatments; planning and managing in-house organizational operations; analyzing relevant data to improve healthcare services; designing contents for web sites and mobile applications to ensure participation to campaigns and informing patients about campaigns; measuring, investigation and improving patient satisfaction; performing risk management and quality improvement activities; training employees; issuing invoices for services; verifying your relation with organizations that have entered into contract with our medical center; answering all your questions and complaints about our healthcare services; taking all necessary technical and administrative measures for data security of our systems and applications; monitoring and preventing unauthorized procedures.<\/p>\n
\u2022 Fulfilling legal and regulatory obligations,<\/p>\n
\u2022 Requesting the information with private insurance firms to check coverage of insurance policies,<\/p>\n
\u2022 Informing you about your appointment through Call Center and our digital channels,<\/p>\n
\u2022 Fulfilling education and training activities by educational institutions cooperating with the institution.<\/p>\n
\u2022 To inform you about our products and services and to send you commercial electronic messages,<\/p>\n
\u2022 Verifying identity details of a person who purchases services over web site and mobile applications; increasing spectrum of services and products measuring thereof and also for legal tracking and similar aims.<\/p>\n
Processes Data and Duration of Storage\u00a0<\/b><\/p>\n
Your below written personal data can be processed for said activities:<\/p>\n
General and sensitive personal data, especially the personal health data necessary for maintaining all medical diagnosis, examination, treatment and care services and obtained for this purpose are listed below;<\/p>\n
Your identity details such as name, surname, R.T. ID number, passport number and temporary R.T. ID number if you are not a Turkish citizen, your place and date of birth, marital status, gender information as well as copy of your R.T. ID Card or Driving License that you have submitted,<\/p>\n
\u2022 Your contact details such as address, phone number, and e-mail address,<\/p>\n
\u2022 Your financial data such as bank account number, IBAN,<\/p>\n
\u2022 Your information about health and sexual life obtained while you receive medical diagnosis, treatment, consultation and care services, such as laboratory and imaging test results, other test results, examination data, prescription data, which you submitted so that they are followed up in your file,<\/p>\n
\u2022 Your responses and comments you share to evaluate our services,<\/p>\n
\u2022 The information you shared voluntarily and your video and voice records obtained during your physical visit to our medical center or your video calls to our physicians,<\/p>\n
\u2022 Your voice recorded during your communication with our Call Center,<\/p>\n
\u2022 Your Social Security Institution data and private health insurance data in order to finance and plan your healthcare services,<\/p>\n
\u2022 Information about your car\u2019s license plate in cases you receive valet parking service.<\/p>\n
\u2022 Your web browsing information, IP address, browser information obtained when you use our web site and mobile applications and medical documents, surveys, form data and location data that you submit voluntarily.<\/p>\n
Neolife will store your information and your sensitive personal data in physical and electronic archives that are located in Neolife and external service providers as long as aim of data processing is valid and for 10 years after aim of processing expires.<\/p>\n
Who can access to your personal information?<\/b><\/p>\n
Your information will be stored in servers of Neolife and your information will be accessed by Neolife and its subsidiaries. Moreover, your personal data and your sensitive personal data pursuant to relevant legislation or your explicit consent:<\/p>\n
\u2022 Ministry of Health, affiliated departments and family medicine centers,<\/p>\n
\u2022 Private insurance companies (health, retirement, life insurance etc.),<\/p>\n
\u2022 Social Insurance Institute,<\/p>\n
\u2022 General Directorate of Security and other law enforcement institutions,<\/p>\n
\u2022 General Directorate of Census,<\/p>\n
\u2022 Turkish Pharmacists\u2019 Association,<\/p>\n
\u2022 Judicial authorities,<\/p>\n
\u2022 Local and abroad laboratories, medical centers, ambulance, medical device and healthcare institutions we are in cooperation with for medical diagnosis and treatment,<\/p>\n
\u2022 Healthcare institution to which the patient is referred or present to,<\/p>\n
\u2022 The legal representatives you authorized,<\/p>\n
\u2022 Third parties that deliver consultancy services to our firm including lawyers, tax consultants and auditors we work with,<\/p>\n
\u2022 Regulation and inspection bodies as well as other legal bodies, such as courts and execution offices; other public bodies and organizations that are authorized to request your personal data,<\/p>\n
\u2022 The business partners, supplies, supportive service providers, and archive service provides that sell products and services or that we cooperate with to deliver and promote our services,<\/p>\n
Shareholders, provided it is limited to design strategies for commercial activities of NEOLIFE, inform them about corporate producedures and to carry out inspection activities in line with provisions of relevant legislation.<\/p>\n
Can be shared with third parties and transferred abroad in order to provide you with diagnosis and treatment services, to fulfill our contractual and legal obligations.<\/p>\n
Your Rights about Protection of Personal Data<\/b><\/p>\n
You have following rights to perform following procedures whenever you wish pursuant to Article 11 of Relevant Law:<\/p>\n
a) Learn whether your personal data is processed or not,<\/p>\n
b) Request relevant information, if personal data is processed,<\/p>\n
c) Learn the intent of processing your personal data and whether they are used as intended,<\/p>\n
d) Learn local and international third parties who are shared your personal data,<\/p>\n
e) Request correction of missing or incorrect data, if any,<\/p>\n
f) Request deletion or disposal of your personal data in line with the conditions mentioned in Article 7 of relevant law,<\/p>\n
g) Request that the operations performed about the third parties, which are transferred your personal data, are reported pursuant to paragraphs (d) and (e),<\/p>\n
h) Object any result as the data that is processed solely on automated systems is analyzed,<\/p>\n
i) Request compensation for damages that arise out of processing your personal data illegally,<\/p>\n

Otherwise, applications will not be taken into consideration.<\/p>\n
The applications will be responded as soon as possible, but not later than 30 days.<\/p>\n
If your application is refused or the response is insufficient according to your opinion or the application is not responded within mentioned period of time, you can submit a complaint latter to Personal Data Protection Committee in 30 days after you get our response to your application or in 60 days after date of application.<\/p>\n
